Schedule a Hospital Visit
Members of the hospital Visitation Team provide support, prayer and encouragement for those in need of a visit during their hospital stay. Daily calls to the Des Moines-area hospitals to provide encouragement help our church family and their loved ones feel encouraged and supported.
If you would like to schedule a call or a visit with our team or the on-call pastor, please complete the form below or call the Care Team at 515-282-4481 or 515-222-1520. Even if you have communicated with the hospital that you would like Hope (and one of its campuses) to be notified, please be sure to reach out to us directly so we can ensure a member of the team stops by!
